Care Coordination provides three levels of service:

1. Intensive for children and adults, Supportive for children and adults, and regular Case Management for adults.
2. AOT case management is available for persons who are considered high risk under Kendra's Law and may be court ordered to receive services.
3. Case Management also provides forensic case management to persons with mental illness who are incarcerated in the County jail.

The components of Care Coordination are: linking, advocacy and monitoring to all service providers and support systems so that an individual's quality of life may be maintained and improved.
