Program Overview

The MOMS Program is a home-visiting nursing program that provides maternal-child health supportive services, free of cost, to residents of Cattaraugus County that are Medicaid eligible. MOMS is designed to improve equitable access to quality maternal child health care, increase lactation support for parents choosing to breast and chest feed, and enhance the coordination of care with other community resources.

Prenatal & Postpartum Services

A Registered Nurse (RN) is assigned to you to provide coordinated case management in conjunction with a Physician and other Community Agencies. 

Your RN provides home visits each trimester to asses status, teach about pregnancy & how to prepare for the babys arrival, as well as visits after the baby is born to teach about baby care, importance of medical follow-up & refer to other community agencies as needed.

  • Health Screening & Monitoring
  • Nutrition Screening & Monitoring
  • Infant Care Education
  • Lactation Counseling
  • Postpartum Mental Health Education
  • Childbirth and Family Planning
  • Early Childhood Development
  • Community Resource Referrals

Breast/Chest Feeding Support

Your RN is also a Certified Lactation Consultant (CLC) who can start you on a path to successful breast/chest feeding and answer any questions you may have about yourself and your newborn. For more information, visit the Office on Women's Health Website

Make your first appointment today. 

If you are a Medicaid eligible pregnant resident and live in Cattaraugus County, you're entitled to support from the MOMS Program.

Call (716) 373-8050 to schedule your first appointment and meet with a Registered Nurse (RN) who will help you with:

  • Pregnancy Education
  • Enrollment into the WIC program (if applicable)
  • Making an appointment with a participating OBGYN

*Participants receive assistance with Medicaid application and if meet criteria receive immediate coverage.
